Please read these SMS Terms and Conditions carefully before opting in to receive text messages from Rad Eyes Mobile / Rad Optometric Care Inc. (“Rad Eyes Mobile,” “we,” “our,” or “us”).

By affirmatively opting in to SMS communications from Rad Eyes Mobile, you consent to receive conversational and informational text messages from us at the mobile number you provide.

By opting in, you confirm that you own the mobile number provided or are authorized to use it.

Consent to receive SMS messages is voluntary and is not a condition of purchasing products or receiving services from Rad Eyes Mobile.

Our SMS messages are used for customer care and may include:

• Appointment scheduling, confirmations, and reminders
• Appointment rescheduling or cancellation updates
• TBI and personal injury referral coordination
• Customer-care conversations
• Care-related follow-ups
• Responses to questions and service requests
• Eyewear order, delivery, and service updates
• Other informational messages related to services you have requested

This SMS program does not include promotional or marketing messages.

Message frequency varies depending on your appointments, referrals, questions, and service needs.

Message and data rates may apply. Standard messaging charges from your mobile carrier may apply to messages sent and received.

Rad Eyes Mobile is not responsible for charges assessed by your mobile carrier.

You may opt out of SMS communications at any time by replying STOP to any message from Rad Eyes Mobile.

After replying STOP:

• You may receive one final message confirming your opt-out request.
• You will no longer receive SMS messages from Rad Eyes Mobile.
• This includes appointment reminders, referral updates, and other customer-care messages.
• You may continue communicating with us by telephone or email.

Rad Eyes Mobile and participating mobile carriers are not responsible for delayed, blocked, or undelivered messages caused by network issues, device settings, carrier restrictions, or circumstances outside their reasonable control.

To the extent permitted by applicable law, Rad Eyes Mobile is not responsible for:

• Messaging or data charges assessed by your mobile carrier
• Delayed, blocked, or undelivered messages
• Technical issues or service interruptions outside our reasonable control
• Unauthorized access to information transmitted through SMS

We may update these SMS Terms and Conditions periodically. Any revisions will be published on this page with an updated effective date.

Changes to these terms do not override your right to opt out of SMS messages at any time by replying STOP.

By opting in to SMS communications from Rad Eyes Mobile, you acknowledge that you have read and understood these SMS Terms and Conditions.

You confirm that you own or are authorized to use the mobile number provided and authorize Rad Eyes Mobile to send customer-care and informational SMS messages to that number.
